We are seeking a motivated and enthusiastic Band 7 Clinical Pharmacist to join our highly successful Primary Care Network in North East London. The ideal candidate will have experience working in primary care. Completion of (or active enrolment on) the CPPE Primary Care Pharmacy Education Pathway (PCPEP) is essential for this role.
As part of a supportive and well‑established Pharmacy team, the post holder will work flexibly across practices in the Network, contributing to the delivery of high‑quality, safe, and efficient patient care. You will support the achievement of the Networks DES and Supplementary Specifications, working closely with Senior Pharmacists, the Clinical Director, and the wider ARRS team to ensure consistent and effective medicines optimisation services across our practices.
Within your scope of clinical competence, you will provide expertise in prescribing and medicines optimisation. Key responsibilities include:
- Conducting structured medication reviews (SMRs) and prescribing independently where appropriate.
- Supporting the management of long‑term conditions through safe, evidence‑based prescribing.
- Undertaking medicines reconciliation following transfer of care to ensure patient safety and continuity.
- Contributing to the development of safer prescribing systems and processes across the Network.

Together, we serve a registered population of more than 53,000 patients, working collectively to deliver high‑quality, efficient, and sustainable primary care. At the heart of our clinical workforce is a dedicated Pharmacist team, comprising three Senior Clinical Pharmacists and two Band 7 Clinical Pharmacists. This team plays a vital role in optimising medicines use, improving safety, and supporting long‑term condition management across our practices. Senior Clinical Pharmacists not only lead by example in clinical delivery but also provide mentorship and line management to Band 7 colleagues, ensuring professional growth and consistency of practice.
Our Pharmacists are fully integrated into the wider Network team and work closely with GPs, nurses, and other healthcare professionals. To provide an equitable and balanced service, each Pharmacist is allocated to a maximum of two practices per rota rotation. This model fosters continuity, collaboration, and quality improvement across the Network while allowing Pharmacists to develop strong working relationships within practices.

Job responsibilities
Structured Medication Reviews & Prescribing
- Hold clinics (face‑to‑face and remote) to deliver structured medication reviews, ensuring medicines are safe, effective, and optimised for individual patient needs.
- Support patients with long‑term conditions and polypharmacy, prescribing and deprescribing where appropriate within scope of practice.
- Request, review, and interpret monitoring tests to support safe prescribing.
- Provide telephone advice and follow‑up for patients with queries or concerns about their medicines.
Case Management & Long‑Term Conditions
- Manage a defined caseload of patients, including participation in long‑term condition clinics.
- Identify and support patients at higher risk of admissions or medicine‑related harm.
- Contribute to multidisciplinary team meetings and case discussions.
- Work alongside care home staff and primary care colleagues to promote safe prescribing, ordering, and administration of medicines.
Repeat Prescribing & Medicines Safety
- Support the repeat prescribing process by reviewing requests for reauthorisation and making changes where appropriate.
- Ensure patients are recalled for monitoring in line with clinical guidance.
- Run clinical searches to identify patients at risk from high‑risk prescribing or unplanned admission.
- Contribute to the development and implementation of safer prescribing systems across practices.
Hospital Discharge & Transfer of Care
- Reconcile medicines after hospital discharge or transfer into care homes, resolving discrepancies where appropriate.
- Produce medicines care plans following discharge, arranging necessary follow‑up and liaising with patients and community pharmacists.
- Work with hospital colleagues to ensure safe, seamless transfer of care for patients with complex medicines needs.
Medicines Optimisation & Governance
- Act on national and local medicines safety alerts (e.g. MHRA), product withdrawals, and new clinical guidance.
- Support the introduction of formulary and guideline changes across practices.
- Assist in developing protocols for managing high‑risk medicines and contribute to staff training.
- Provide medicines information and advice to GPs, nurses, and other members of the practice team.
Public Health & Specialist Support
- Support the delivery of national and local public health campaigns within the PCN.
- Provide specialist clinical input in areas such as immunisation and long‑term conditions.
Collaborative Working
- Work effectively as part of the multidisciplinary team, recognising personal limitations and referring to senior colleagues where necessary.
- Develop and maintain effective working relationships across the PCN, community pharmacy, hospital pharmacy, and wider health and social care system.
- Liaise with ICB pharmacists and Medicines Optimisation teams to support consistent, evidence‑based approaches to prescribing.
- Communicate effectively with a wide range of stakeholders including GPs, practice staff, community teams, allied health professionals, and secondary care colleagues.

Person Specification
Qualifications
- Registered pharmacist with the General Pharmaceutical Council (GPhC) in the UK.
- Member of the Royal Pharmaceutical Society (RPS) or working towards membership.
- Completion of the CPPE Primary Care Pharmacy Education Pathway (PCPEP) (or prepared to undertake this as part of your role).
- Evidence of being actively on route to independent prescribing with a clear timeframe for completion.
Experience
- Proven experience working in primary care/general practice environment.
- Experience conducting structured medication reviews (SMRs) and managing polypharmacy and long‑term conditions.
- Demonstrable experience in medicines reconciliation, particularly around transitions of care (e.g., hospital discharge, care home transfers).
- Solid understanding of medicines optimisation, safe prescribing principles, and management of high‑risk medicines.
- Knowledge of national and local prescribing guidance, formularies, MHRA safety alerts, and ability to act on them.
- Competence in independent clinical decision‑making and safe prescribing within scope.
- Ability to interpret and act on monitoring results and adjust therapy accordingly.
- Skilled in using primary care IT systems (e.g., EMIS Web, SystmOne or equivalent) for patient review, searches, and documentation.
- Strong communication skills, both verbal and written, to engage patients, clinicians, and wider stakeholders.
