Senior Clinical Nurse - Patient Solutions

Make a difference in patients' lives while leading and supporting a dedicated clinical team.
We’re looking for an experienced Senior Clinical Nurse to join our Patient Solutions team, delivering outstanding care as part of the Baxter Homecare Programme.
The Role
- Lead and support a small team of nurses (up to 5).
- Deliver safe, effective patient care in homes, hospitals, and virtually.
- Build and maintain strong relationships with patients, carers, NHS hospitals, and stakeholders.
- Provide patient education and transition support between hospital and community care.
- Manage performance against KPIs, audits, and service standards.
- Deputise for the Nurse Manager and contribute to on‑call services.
About You
- NMC Registered Nurse with 2+ years’ post‑registration experience.
- Strong leadership and mentoring skills.
- Excellent communicator with a patient‑centred approach.
- Full UK driving licence and willingness to travel.
- Desirable: experience in CVAD, parenteral nutrition, or related therapies.
