Overview
We are looking for an Advanced Clinical Practitioner for a rotational post within the Virtual Hospital, supporting the General Medicine virtual ward across multiple partner sites. The role requires clinical leadership, governance, and direct patient care delivery to reduce hospital admissions.
Key Responsibilities
- Lead clinical service delivery and govern the virtual ward programme.
- Provide patient care at their place of residence, including monitoring, interventions and management.
- Collaborate with multidisciplinary teams across West Hertfordshire (Watford General site) and the Central London Community Hospital Trust (BRE building, Bricket Wood).
- Travel to all assigned sites and drive patient appointments within the geographical catchment area.
- Support the four pillars of advanced practice: expert clinical practice, professional leadership, education/training, and research/evaluation.
Working Pattern
8:00‑20:00 shifts with 1 in 4 weekend shifts. The role is fully rotational, requiring travel between partner organisations.
Qualifications and Experience
- Band 7 clinical practitioner with experience in acute or community settings.
- Demonstrated ability to lead and manage care programmes.
- Commitment to completing required competencies for progression to Band 8a.
- Knowledge of virtual ward models and digital monitoring technologies is desirable.
