We are seeking a passionate IP ClinicalPharmacist with experience of working in general practice to join our dynamic team.

Ourcurrent team is made up of 17 pharmacists and 5 pharmacy technicians, and wepride ourselves on our friendly and supportive team ethos. We have excellent, robustclinical governance procedures in place with a team of senior clinicalpharmacists and a GP lead providing regular clinical supervision, team meetingsand training. Rushcliffe PCN consists of 11 practices which are all fullyengaged with the PCN pharmacy team and provide supportive integration of these rolesinto practice. PartnersHealth is the GP federation for Rushcliffe and theemployer for all additional roles within the PCN.

Main duties of the job

The clinical pharmacist will work as an independent prescriber within their clinical competencies to provide expertise in clinical medicines management across a range of aspects such as day to day medication related queries, acute prescription requests, supporting hospital discharge prescribing arrangements and follow up, actioning of MHRA alerts and targeted medication reviews which may be done over the telephone or face to face. You will be expected to help in streamlining processes and developing protocols to allow for safer prescribing and to meet CQC expectations.

Youwill act as a specialist resource to answer medicines-related queries, tosignpost to other services and to liaise with other healthcare professionals asrequired. Clinical supervision and mentorship will be provided by the clinicallead GP and the senior clinical pharmacist.

Weare looking for candidates who are enthusiastic self-starters and who areadaptable to change. You will have excellent communication skills and beproactive in approaching patients, carers, colleagues and other agencies as andwhen needed. It is expected that you will be able to use your initiative togather information and to use the resources that are available to you, such asprescribing guidelines. Being a team player is essential. A good level ofcompetence in SystmOne is required.

Job responsibilities

THE PURPOSE OF THE ROLE

General Practice is at the heart of the Healthand Social Care system, and it is critical to have thriving, sustainablegeneral practices that meet local peoples health needs. The PCN ClinicalPharmacist is instrumental in supporting their own practices clinically. Thisis a key role, intended to support the delivery and development of thepharmacist role in primary care andensure that we continue to provide high quality, accessible and responsiveservices to both practices and patients.

KEY RESPONSIBILITIES

Actswithin professional boundaries, supporting and working alongside a team ofpharmacists and pharmacy technicians in general practice.

Works as partof a multi-disciplinary team in a patient facing role.

Takesresponsibility for areas of chronic disease management within the practiceand undertakes clinical medication reviews to proactively manage patientswith complex polypharmacy.

Providesprimary support to general practice staff with regards to prescription andmedication queries. (helps support the repeat prescriptions system, dealwith acute prescription requests, and medicines reconciliation on transferof care and systems for safer prescribing, providing expertise in clinicalmedicines advice while addressing both public and social care needs ofpatient in the GP practice)

Providesclinical leadership on medicines optimisation and quality improvement andmanages some aspects of the quality and outcomes framework and enhancedservices.

Ensures thatthe practice integrates with community and hospital pharmacy to helputilise skill mix, improve patient outcomes, ensure better access tohealthcare and help manage workload.

The role ispivotal to improving the quality of care and operational efficiencies sorequires motivation and passion to deliver excellent service withingeneral practice.

This post is subject to a Criminal RecordsBureau disclosure. The post holder is expected to adhere to the professionalguidelines and codes of conduct of the General Pharmaceutical Council.

RiskManagement

The post-holder will ensure compliance withthe PCNs risk management policies and procedures and demonstrate a commitmentto risk management, the recognition that our aim is to protect patients, staffand visitors from harm, and stress that all staff have a responsibility tominimise risk.

GovernanceStandards

Comply with the relevant governance standardsapplicable to the PCN and as communicated to the post-holder from time to time.

DataProtection

To comply with PCN Policies and the DataProtection Act in all respects, with particular relevance to the protection anduse of patient information.

Confidentiality

The PCNs employees are required to ensurethat information about patients is safeguarded to maintain confidentiality andis kept securely in accordance with NHS requirements. This means that a patient's information can only be passed to someone else with the patientsconsent or if it is essential to the effective provision of care for thatindividual.

Child/Adultprotection

All staff must be familiar with and adhere tolocal and Statutory child protection procedures and guidelines, in conjunctionwith the Nottinghamshire Area Child/Adult Protection Committee policies andprocedures. All staff are required to attend child/adult protection awarenesstraining and additional training regarding child protection relevant to theirposition and role.
