Community Mental Health Practitioner

To work as part of the Bridport Community Mental Health Team. To be responsible and professionally accountable for a patient caseload within the community. The postholder will be working with people with a serious mental illness, providing high‑quality nursing care and treatment alongside the multi‑disciplinary team.

Main duties of the job

The postholder will be responsible for a caseload in the community, undertaking bio‑psychosocial assessments, covering the duty rota and providing evidence‑based interventions with their patients.

If the postholder is a registered nurse, they will be rota'd on to the Depot clinic and also support the nursing team with taking bloods and be part of managing the Clozaril Clinic.

Employment requirements

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Driving is essential for this role. You will be required to have a full and valid UK driving licence and to travel using your own vehicle on Trust business.
