Hospital at Home Urgent Care Response Advanced Assessor Nurse

This post represents an exciting opportunity to join our Hospital at Home (H@H) service as a Band 7 Nurse, working as part of a dynamic and innovative multidisciplinary team delivering acute‑level care within patients' own homes and usual place of residence. Underpinned by CNWL's core values of compassion, respect, empowerment and partnership, and our cultural framework SCARF – Safe, Compassionate, Accountable, Reflective and Fair, the post holder will provide senior clinical leadership across the Hospital at Home pathway, working as an adjunct to the Urgent Community Response (UCR) service, supporting admission avoidance and early supported discharge across Hillingdon Borough.
The post holder will lead on the clinical assessment and management of patients with complex and acute needs, including those with established frailty, who would otherwise require an acute hospital admission. You will be responsible for making advanced, autonomous clinical decisions, overseeing daily MDT reviews, and ensuring that all patients receive safe, high‑quality, person‑centred care within a maximum 14‑day episode. You will provide professional leadership, supervision and mentorship to Band 6 and junior colleagues, and act as a key point of escalation for clinical concerns across the team.
Main duties and responsibilities
- Close collaboration with GPs, Consultant Geriatricians, acute trust colleagues, primary care, pharmacy and social care embodying CNWL's vision of Wellbeing for Life through genuine partnership working.
- Demonstrate highly developed clinical competencies including Comprehensive Geriatric Assessment (CGA), point of care diagnostics, IV therapy, medicines management and end of life care planning.
- Play a central role in shaping the development of the service, contributing to governance, audit, quality improvement and the ongoing development of clinical pathways in line with NHS England national priorities for virtual wards and Hospital at Home.
- Assess, plan, implement and provide evaluation of health care needs in conjunction with patients, their carers and other appropriate members of the multidisciplinary team.
- Lead across the team, provide nurse leadership and management support to the team.
- See patients with potentially undifferentiated diagnosis including objective assessment, physical assessment, treatment, follow‑up and development of care plans.
- Ensure all patients on the caseload are managed well and discharged safely either to their GP or for further ongoing assessment, rehabilitation, treatment and care.
- Keep up to date with current issues and events in Primary and Secondary Health Care.
- Liaise with community services partners.
- Take a supportive lead role in the development of the Rapid Response Team(s) and integrated team working with other services.
- Provide support and health education for patients and carers through effective interpersonal skills.
- Support team members to comply with professional codes of conduct and trust organisational policies and procedures and the implementation of performance management when required.
- Work flexibly as required and requested by the line manager to meet the needs of the service.
- Ensure own mandatory training and junior nurses' mandatory training is kept up to date in line with Trust policies.
- Undertake comprehensive assessment of health and social care needs using appropriate assessment tools to achieve Person Centred Care.
- Undertake advanced practitioner level physical examinations of patients needing complex chronic disease management and resolution or containment of disease complications.
- Promote good practice to improve standards of care.
- Recognise early symptoms of disease exacerbation, acute illness and injuries, based on understanding of chronic diseases, disease processes, current evidence and practice standards.
- Make direct referrals for medical assessment and diagnostic procedures.
- Demonstrate a high level of clinical judgement, acting autonomously in a variety of contexts (acute and primary care).
- Discuss and agree assessment outcomes with patients, carers and other health care professionals, enabling them to plan risks to health and wellbeing.
- Delegate appropriately, ensuring staff are competent to undertake delegated duties and tasks.
- Recognise own limitations and seek help/guidance when appropriate.
- Prevent unnecessary hospital admissions by responding to LAS, GPs, community colleagues and actively identifying patients who would benefit from Rapid Response Team intervention and out‑of‑hospital services.
- Participate in the development and evaluation of teaching programmes for students on community programmes and junior staff.
Person specification
- Extensive experience in appropriate speciality.
- Mentorship or teaching qualification equivalent to ENB 998 / workplace assessor.
- Non‑Medical Prescriber or working towards.
