The Jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care. The jobholder will provide specialist nursing care and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources. The post holder is primarily involved in the assessment and management of patients through the local stroke and TIA care pathways as well as manage the regional thrombectomy referrals. This role involves undertaking competency based patient assessment and examination in order to formulate a management plan for specific groups of patients admitted for diagnosis and/or treatments. You are expected to undertake all preparatory work prior to the patient undergoing treatment, for example the request of any further investigations and to highlight any potential complications to the treating clinician.

An exciting opportunity has arisen for an experienced nurse to join the existing Brain Attack Team within the Stroke Services at LTHT, supporting the assessment and delivery of hyper acute stroke care.
You will spend a considerable amount of time in the Emergency Department, supporting the stroke team in the assessment of patients and through the stroke pathway. Successful candidates will be expected to support assessment of patients who may be eligible for hyper acute stroke interventions, alongside the management of TIA's and stroke mimics, you will undertake further training to support this.
You will be expected to participate in audits and research to help develop and shape the future of stroke care in Leeds and the wider region. You will provide a high standard of clinical leadership, supporting the delivery of evidence based, patient centred care.
You will be part of a busy multi-disciplinary team, who are dedicated in providing individualised, evidence based stroke care management. The post holder will be expected to work both autonomously and closely with stroke clinicians and across a range of specialities in order to meet the needs of the service. You must be a competent communicator, able to gather comprehensive history. Ultimately we are searching for a dynamic, highly motivated and passionate individual committed to high-quality patient care.

We admit over 1300 strokes a year to our hyperacute stroke unit and offer a challenging and dynamic clinical learning environment dealing with a wide range of stroke patients, TIAs and stroke mimics. This includes thrombolysis, thrombectomy and management of primary intracerebral haemorrhage as well as the delivery of a regional thrombectomy service for West Yorkshire.
