Greater Manchester Mental Health NHS Foundation Trust – Bolton Neighbourhood Mental Health Service – full‑time post available as a Registered Mental Health Nurse, Registered Social Worker, or Occupational Therapist.
Role Description
The postholder will undertake a key worker role and an assessment and formulation role within the multidisciplinary team. They will triage and signpost referrals, liaise with agencies and professionals, and support the service’s duty rota.
Postholder will be based in Farnworth or Horwich Hubs. The service operates 9am–5pm Monday–Friday; duty rota may require work until 7pm and occasional weekends.
Key Responsibilities
- Key worker duties, providing timely access to services.
- Assessment and formulation of service users.
- Triaging and signposting referrals.
- Liaising with community services and third‑sector organisations.
- Participating in a duty rota, including on‑duty at the Referral and Assessment Hub and potential weekend work.
Qualifications & Experience
- Registered Mental Health Nurse, Registered Social Worker or Occupational Therapist (preferable).
- Experience in triaging and signposting referrals.
- Knowledge of community services and third‑sector organisations in Bolton.
