We also collaborate with the Health and Social Care Partnership and social work to ensure patients’ journeys and discharges meet their needs.
Supported Discharge Unit care is for patients with complex health and social care needs, including end‑of‑life care patients.
The patient discharge journey should start from admission, and the Supported Discharge Unit plays a key role in supporting hospital capacity through timely and safe discharges.
It is essential that candidates can demonstrate strong communication skills.
A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme prior to appointment.

Candidates should confirm that they have an appropriate right to work in the UK or that the post would be eligible for sponsorship before submitting an application.
