Thepostholderwillworkcollaborativelytoensurepeoplelivingincarehomeshaveaccesstoenhanced primarycareandtospecialistservicesandmaintaintheirindependenceasfaraspossiblebyreducing, delaying or preventing the need for additional health and social care services
Responsibilities
- Toidentifyandaddresshealthinequalitiesandensurethatlong-termhealthconditionsare identified and addressed
- Toassess,adviseandcontributetomeetingthehealthneedsof peoplewithcomplexphysical health
- IntroduceaTrainingAnalysisof residentialcarehomestafftoidentifyareasoffurther development and educative support.
- Promoteandsupportup-takeofAnnualHealthCheckswithGPPractices/PCNnetworks
- Review quality of Health Action Plans/Health Care Passports – Advise/support/facilitate identifiedhealthneeds
- Supporttrainingandeducation
- LiaisewithGPservicestoformulateandimplementUrgentCarePlanninginline withcurrent guidance. Advocate for residents.
- Toworkwithpeoplewhoengageinbehaviorsthatchallenge(e.g.physicalaggressionto others, self-injurious behaviors etc.)
- Supportstaff withMCA/BIdecisionmakingwithGPservices/involvedhealthprofessionalsandfamilies.
- Supportcarehomeshospitaladmissionandcomplexdischarge planning.
- EnsurethatcarehomestafffeelsupportedandincludedaspartoftheMDTandintegrated professional network
- ProvideregularreportsforLeDeRviatheLACandsteeringgroup.
- Tocontributetomulti-disciplinaryplanningandmeetings
