Overview
The Palliative Care Clinical Nurse Specialist will provide in‑reach services within the acute hospital areas of Calderdale & Huddersfield NHS Trust (A&E, Acute floor, and SDEC). Working alongside a designated palliative care social worker, the role involves identifying and facilitating the rapid discharge or transfer of patients with palliative care needs to an appropriate preferred place of care, and supporting timely, effective care, assessment, and intervention in a multi‑professional workforce.
Clinical Duties
- Work within the multi‑professional team to manage the caseload of patients presenting with specialist palliative care needs in the acute setting (Emergency Department, Acute floor and SDEC).
- Tri‑age and holistically assess patients with life‑limiting conditions, facilitating rapid discharge to the preferred place of care where appropriate.
- Provide symptom‑management advice.
- Tri‑age, assess and, when necessary, escalatE patients to the Hospital specialist palliative care team.
- Work alongside an allocated palliative care social worker to facilitate social care and FT‑funded needs.
- Liaise with community palliative care teams and other multi‑professional teams to support patients' needs and ensure admission avoidance in acute setting and from home.
- Act as a resource for signposting patients, families, carers, and health professionals to palliative care services in hospital and community.
- Provide guidance and support for patients making choices regarding preferred place of care and advance care planning.
- Involve patients, families and carers in planning and delivery of care.
- Assess and facilitate rapid assessment to hospice when appropriate.
- Support advance care planning and record in System One EPaCCS when consenting to share information.
- Support integration of end‑of‑life care tools (e.g. SPICT / Gold Standards Framework).
- Audit performance of the project to participate in service review and development alongside the Macmillan palliative care team leader.
Professional Duties
- Work within the NMC Code of Conduct at all times.
- Ensure adherence to all CHFT protocols, policies and directives within service delivery.
- Act as a role model for quality nursing care, promoting evidence‑based interventions and compassionate care.
- Demonstrate specialist skills and knowledge.
- Act as a patient advocate at all points of the care pathway.
Managing Self
- Participate in regular supervision.
- Attend all mandatory training.
- Identify, develop and agree a personal development plan with your line manager annually.
- Comply with all Trust policies, procedures and protocols.
- Maintain professional conduct, including appearance.
- Ensure maintenance of Professional Registration.
- Seek advice and support from line manager whenever necessary.
- Carry out duties with due regard to the Trust’s Equal Opportunity Policy.
Managerial Duties
- Participate in service review and development alongside the specialist palliative care team.
- Ensure continuity of care for patients and access to specialist nursing or other health and social support across care settings.
- Work with disease‑specific clinical teams to promote equity of access to specialist palliative care for malignant and non‑malignant conditions.
- Support CHFT’s service delivery aims and targets.
- Take part in weekly clinical review meetings to discuss complex cases and reflect on outcomes.
- Promote excellence in palliative care through evidence‑based practice and measurable standards of care.
- Identify and highlight gaps in evidence or service provision under supervision of the Macmillan specialist palliative care nursing team leader.
- Attend relevant internal and external meetings and provide effective communication within the team.
- Maintain a flexible approach and develop the role as needed.
- Support the Trust’s annual plan and strategic vision in all care settings.
- Support the cost‑efficiency programme by correct use of resources and identifying areas for improvement.
