Critical Care Outreach Nurse
We are looking for an experienced Critical Care Nurse or equivalent to join our Outreach Team, providing expert care, advice, and support to patients at risk of deterioration or those who are critically ill – as well as their families and carers.
Key Responsibilities
- Support patients until they are either transferred to the Critical Care Unit, their condition improves, or it is agreed that continuing this level of intervention is no longer in their best interests.
- Provide early intervention with appropriate assessment, observation and treatment of seriously ill patients in general ward areas (level 1 and level 2 patients).
- Act as a source of expert advice and support to those caring for deteriorating or critically ill patients and their families on the wards.
- Work autonomously, making clinical judgements, managing own workload, and seeking advice from senior Critical Care nursing and medical staff as necessary.
- Promote the nursing contribution to the Directorate’s and Trust’s clinical governance plans.
- Undertake projects, audits and research to improve deteriorating patient/high‑dependency care skills across the Trust.
- Provide multi‑disciplinary education in deteriorating/high‑dependency care, including development of doctors in training and qualified nursing staff.
- Undertake Trust‑agreed advanced specialist clinical practice skills to meet patient needs.
- Assist with the development of the Critical Care Outreach Team (CCOT).
Qualifications
Role requires an experienced Critical Care Nurse or equivalent, with strong clinical judgement, multi‑disciplinary collaboration skills, and a commitment to high standards of patient safety and outcomes.
