Job Summary
The post holder will work in conjunction with members of the multidisciplinary Heart and Lung Transplant/VAD (left ventricular assist device) team to coordinate the nursing aspects of the patient pathway, pre and post heart and lung transplantation and mechanical circulatory support insertion. The role is a Band 6 position.
Key Responsibilities
- Supporting the post‑transplant recipient's preparation for discharge by engaging in teaching and education of immunosuppression post‑transplant, completing discharge talks, and attending daily ward rounds and multidisciplinary team meetings.
- Running post‑heart or lung transplant clinics.
- Completing post‑op clinic follow‑up calls.
- Completing post‑transplant and LVAD national database forms.
- Providing LVAD education and supporting LVAD clinics.
- Participating in drive‑line wound multidisciplinary team meetings.
- Initiating inpatient assessments and completing assessment bloods and urine samples.
- Engaging in pre‑transplant counselling talks.
- Acting as a specialised practitioner within scope of practice, managing a caseload of patients, delivering individualised direct patient care within a multidisciplinary team.
- Managing LVAD patient education and care pre and post implant, including ward‑based education, driveline MDT preparation and attendance, and supporting outpatient LVAD clinic.
- Managing post‑transplant heart and/or lung patient education and care, including self‑medication education, discharge preparation and running an outpatient clinic.
- Providing written and verbal information during assessments.
- Attending daily ward rounds for all subspecialty areas.
- Supporting clinic preparation and communicating with patients, relatives, medical staff and multidisciplinary team members verbally and in writing.
- Maintaining accurate, legible, timely documentation in accordance with Trust standards.
- Assisting with clinical development and improving governance.
Qualifications
- Registered adult nurse on the NMC register.
- Recognised mentorship course.
- Post‑registration CPD or qualification in an appropriate specialty or professional knowledge, supplemented by specialist experience and short courses.
Experience
- Substantial experience in managing an acute ward or area.
- Minimum 2 years of cardiology/respiratory experience and awareness of the conditions.
- Experience working with acute and chronic diseases.
- Experience working autonomously.
- Commitment to high quality patient care.
- Experience developing educational programmes, teaching, assessment and supervising others.
- Knowledge of relevant NHS policy context.
- Clear understanding of the nurse specialist role.
- Evidence of implementing research‑based practice.
- Willingness to teach at local and national level.
- Experience supporting others in a clinical environment.
- Desirable: substantial experience of working at Band 5 or above in acute cardiac/pulmonary nursing; counselling/communication skills; clinical audit knowledge; monitoring standards; implementing change; research initiatives; corporate and clinical governance knowledge.
