Our client, a Great Manchester Council, is seeking an Adults Social Worker to join their Hospital Discharge Team.

Responsibilities of this role:
- Complete Care Act assessments and support planning for individuals preparing to leave hospital.
- Work closely with health professionals, discharge coordinators, and community services to facilitate smooth transitions.
- Identify risks and safeguarding concerns, implementing robust plans to protect vulnerable adults.
- Arrange care packages, placements, and community support to promote independence and well-being.
- Ensure compliance with the Mental Capacity Act and best interest decision-making where required.
- Maintain accurate records and contribute to multi-disciplinary discharge planning meetings.
- Respond promptly to urgent cases and manage competing priorities in a fast-paced environment.
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have minimum of 3 years post qualifying social work experience
