Weare delighted to offer an exciting opportunity for a motivated and passionate Vascular Specialist Nurseto join our dynamic and expanding Vascular Team at South Tyneside andSunderland NHS Foundation Trust.
Thisis a key role in delivering highquality vascular care across both hospitalsites, supporting patients with complex vascular conditions and contributing tothe development of our surveillance, leg ulcer, arterial and venous services.You will work closely with Consultants, ACPs, and the wider multidisciplinaryteam to provide specialist assessment, support treatment pathways, and enhancepatient experience.
Main duties of the job
- Deliveringspecialist vascular assessments, planning care, and supporting treatmentdecisions.
- Runningand supporting nurseled clinics (including leg ulcer, varicose vein andsurveillance clinics).
- Supportinghot clinics and urgent reviews.
- Providingexpert advice and education to patients, carers and colleagues.
- Contributingto inpatient reviews, ward rounds, and crosssite vascular support.
- Supportingvascular surveillance pathways and ensuring timely followup.
- Workingcollaboratively within an experienced multidisciplinary vascular team.
- Participatingin service development and quality improvement projects.

The Clinical Nurse Specialist (CNS) will work within theexisting CNS team for Vascular Surgery, leading the management and caredelivery for vascular patients with emphasis on prevention, screening andsurveillance of vascular disease, alongside time critical vascular careprovision.
As a CNS within the Vascular services,the post-holder will:
- Provideclinical expert knowledge to patients.
- Supportpatients and their carers when diagnosed with vascular disease.
- Contributeto the ongoing development and delivery of nurse led services for patients.
- Demonstrateleadership in contributing to a culture of patient safety and continuousimprovement in line with the clinical governance framework.
- Beaccountable for the assessment, planning and delivery of all aspects of patientcare across the pathway.
- Provideeffective supportive care to assist patients living with vascular ensuring thatcare is personalised and the holistic needs of patients are met.
- Reducelength of hospital stay and unnecessary hospital admissions.
- Toenable patients to spend a greater proportion of their time within their own homes.
- Toensure all investigations are appropriate therefore reducing unnecessaryprocedures that offer limited value to patient outcome.
- Tosupport the existing vascular CNS team to deliver support to vascular patients.
- Toprovide inpatient and outpatient and community support to patients.
- Todeliver a flexible approach which meets the requirements of the service.
- Providedirect access for patients, inpatients or within the community, either face toface or telephone consultation.
- Provideeducation and training and clinical advice and support to Trust staff, patientsand relatives/carers.
- Participatein research relevant to the needs of specialist area.
- Participatein service development within vascular services across the Trust.

Person Specification
Experience
- Clinical experience within Vascular services.
- Experience of leading within a team.
- Evidence of innovation in practice.
- Recent experience of having facilitated learning in practice.
- Evidence of continuing professional development.
Qualifications
- Degree in nursing studies or other relevant clinical degree. or evidence of currently working towards this (UK equivalent).
- Mentorship qualification/equivalent and or Practice Assessor training or demonstrable equivalent experience.
- Clinical skills accreditation. (May apply without clinical skills but post will be offered as band 6 training post until competence achieved.)
- Non-medical prescribing or willingness to complete.
- Qualification in specialty.
- Masters level education in related subject.
Skills and Knowledge
- Awareness of national health and social care policy.
- Knowledge of best practice in area of specialism.
- Thorough understanding of expanded roles/professional practice issues.
- Clinical teaching experience.
- Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassurance skills are required.
- Able to work without supervision.
- Competent in the use of IT packages.
- Evidence of competency in relation to nursing practice appropriate to the post.
Physical skills
- Ability to assist patients to move as per Trust Moving and Handling Policy.
- Able to meet the travel requirements of the post.
